Please return the Basic Registration Form and the Commitment Page to the rectory to register your child(ren) for this year's program.
If you have any additional questions about program, please call the rectory at (773) 725-7641 and ask to speak to Christopher Perez, the Director of the program and he will be happy to let you know about further informational meetings that will be taking place over the summer through which you may register your family for the program.
